I calibrated an instrument, checked it against itself, and reported 8.5 mV. The real error was 521 mV.

A voltage source's calibration table measured within 8.5 mV of target. Every point passed. The check and the table were both built from the same 12-bit ADC, so the check could not see the error it shared.
